As a Patient Advocate and Community Liaison at Mission, where I have worked for the past 6 years, it is quite literally my job to help ensure patients are receiving the care they need at home. My team is extremely knowledgeable and comprised of amazing individuals, from the clinical hands on patients in their home setting, to the hands behind the scenes ensuring there are no delays in care when our patients need it most. We support our local hospitals, senior care facilities and many other types of Healthcare providers with local community-invested staff to keep patients healthy or healing in the comfort of their own home. Our patients are often our friends, family, neighbors. There is no road map for how to navigate caring for aging loved ones or what happens to us during/after a major health incident, but I dare say (because if I didn't think so, I would work somewhere else)... we're as close to a road map as you can get.
